Become a Patient of the Emory Adult Congenital Heart Center (EACH)
If you have a history of congenital heart defects (CHDs) or have recently received a new diagnosis, you have two options for scheduling an initial consultation at the EACH Center:
- Ask your primary care or other treating physician to make a referral.
- Call 404-778-5036 directly to schedule an appointment for yourself. You should also fill out our Patient Self-Referral Form (PDF 104 KB) and fax it, along with pertinent medical records, to 404-778-5035.
The following information will expedite the scheduling process for new patients:
- Patient’s name, address and telephone number
- Patient’s date of birth
- Patient's social security number
- Referral from the patient’s primary care physician to facilitate insurance approval
- Primary care physician's name, address and telephone number
- CHD diagnoses and date of initial diagnosis
- Description and records of any previous surgeries, including dates and facilities where the procedures were performed
- Diagnostic testing reports and electronic copies of imaging results, including echocardiograms, magnetic resonance imaging (MRI) scans, computed tomography angiograms (CTAs), cardiac catheterizations, stress tests and electrocardiograms (EKGs)
- Insurance information
First Consultation
Your initial visit will include an evaluation based on current symptoms and conditions and compilation of a thorough medical history. Please bring all pertinent medical records and diagnostic testing results with you to the first appointment (see list above). If possible, our physicians prefer to review these items prior to the consultation. This can be arranged by calling 404-785-6330.
